The Nathaniel Witherell is adding a new service to support the Greenwich seniors it serves — Witherell At Home, in-home care for its newly discharged short-term rehabilitation residents. The new service comes through a partnership between The Nathaniel Witherell and DanielCare Caring Choice, LLC, a leading home care organization serving the Fairfield County community.

“We are pleased to be able to offer this much-needed new service to our clients,” says the Witherell’s Executive Director Allen Brown. “We have a long-standing relationship with the team at DanielCare, and feel they are the best organization to extend our commitment to excellent care to the home setting. This new partnership lets us expand our services beyond the walls of the Witherell so that our seniors are well-cared for once they are back home, too.”

When residents leave The Nathaniel Witherell’s short-term rehabilitation unit, they are often still in need of temporary assistance for home and personal care. The Nathaniel Witherell’s staff can arrange and coordinate the highest quality home and personal care at discharge through Witherell At Home, to be executed by DanielCare Caring Choice.

“We’ve worked with The Nathaniel Witherell for many years, and welcome the opportunity to make our flexible and highly-personalized care available to its clients,” says Daniel Kraus, executive director of DanielCare Caring Choice. “Our approach to home care is very synergistic to the person-centered care provided by the Witherell, so clients will continue to get the same individualized care they have come to expect in the comfort of their own home.”

Witherell At Home services include 24-hour live in care, homemaker and companion care, meal preparation, light housekeeping, laundry, errand services, grocery shopping, transportation services, recreational activities, grooming and dressing supervision, and more as needed. All DanielCare Caring Choice caregivers are bonded employees, carefully screened and monitored to provide safe, reliable, and high-quality services in-home. After an initial in-person home assessment conducted by DanielCare’s executive director and a field supervisor, a caregiver is introduced to ensure that a perfect match has been made.

“Quality at-home care is important to a successful recovery,” says Allen Brown. “When our clients know that they will be safe and supported at home, it lets them focus on the important task of healing and recovering.”
